Default immunisation record

i recently had my tetanus, diptheria & MMR jabs at my local quacks and i have my
immunisation record from birth which has been filled in by various doctors throughout
my childhood and recently the practice nurse complete with her signatures . My
question is will my immunisation card be enough or do i have to get some kind of
certifficate of official letter from them.

kind regards

All I had was a scrappy-looking photocopied sheet of that part of my & my kids'
records that listed immunisations. All 3 sets of records were photocopied together
onto a single page. It was ok. (London UK)

The nurse at the medical exam told me that she needed immunization records with the
dates of when I was given them. As long as they have the dates I'm sure it will be
fine. I went ahead and contacted the health department where I live and they gave
me an official immunization record (FREE) and that seemed to work great for me.
Best of Luck

On thorough investigation by both my doctors and the local health authority it turned out that prior to 1995 (I'm 37) they had no records for me ,they contacted previous areas that I had lived in to no avail.

Once I'd convinced the local docs for the need to give me the jabs the nurse wrote what vaccinations she had given me on headed notepaper which I presented at the medical and that was acceptable.
