Default HELP PLEASE URGENT

My wife entered the U.S on a K1 visa.
We married three weeks ago.
I am gathering the documentation, forms, etc. for her AOS.
One document needed is the vaccination supplement from her
medical exam. We have this document.
The doctor signed and dated the document.
Her name, case number, etc. is filled in, but the vaccination history
section is BLANK.

Is this going to be a problem when we send in her I-485 application?
Or, is it common to leave the vaccination history blank?
We are not the only ones as we saw others with theirs blank too.
If it will be a problem, what can we do?
Should I fill in the vaccination history from her shot records?
Should we just send copies of her shot records with the I-485?

Is there anyone here who has gone through the same problem?

Advice is greatly appreciated..

Not an authority on the subject, but seems that the best thing to do is send
copies of shot records with it.

JM,

If it's blank, send in your shot records but do not fill out the vaccination
form; as a copy was sent separately to the service center and it won't match
the one you submit. They send the separate copy to the corresponding center
based on your address and wait for your submission to catch up.

My wife did hers in London prior to her K-1 interview. Everyone else
skipped the vaccination during their medical exam. I asked her to wait
because it would be easier to get it done then and have it documented.
Apparently everyone else's is blank because they opted not to wait and skip
the shots. She was anxious and crying because she was the only one left in
the waiting room. But it did pay off in the end.
