Give the married name, and current address. At least, that's what I did.

I beg to differ with the other poster.

If you are talking about the block that says 'Husband or Wife, if none
so state', the block specifically asks for maiden name of wife.

A similar instruction applies to the block that says 'Former Husbands or
Wives.'
