Guys,
I need help here. The Consulate can´t send me a copy of this form. And I have no clue
which one is it. I couldn´t get at internet in any of the pages posted.
Does anyone has or know about this especifically form?

It's Ok. The American Consulate in Rio de Janeiro will try to send this form to me. As
soon as it arrives I will tell you what is that about. Thank you, and my apologies for any
