Form I-693
I have a question about form I-693. At the time of obtaining my K1 visa to enter the States, I had a medical in London but required one extra vaccination which I got done at my local GP’s office. I was given various forms by the office in London and then had my entire vaccination history printed by my GP.
Now, I had my green card interview but they wouldn’t accept the paperwork I had been given and have now said I need an I-693 form signed off by a civil surgeon.
My question is, will I need to go through the entire medical process again? Or will it be a case of me taking paperwork to the surgeon and have them complete it based on the information I already have? (including CD copy of my chest X-ray). Any idea on what this will cost me as well?
Any help would be appreciated.

You shouldn't need a whole new medical. You probably just need a USCIS approved civil surgeon to sign of that you've had your vaccinations. They don't need to see your xrays.
Cost is set by the doctor, so could range from $25 to $125 or so.

CD is a souvenir.
First you need to find a Civil Surgeon who will just complete the I 693 and just charge you for that.

Yep, just had ours two months ago. Needed to find an approved Doctor (use the USCIS website) and then they took the medical records plus had to do a couple of blood tests. Cost us $300 each for about an hours work in total for four of us.
