Default Expired Immigrant Visa (IV) - Anything to do?.

Hello,

A relative was granted an IV to enter the U.S., but never made it
during the validity of the visa due to a variety of reasons. Some of
the reasons could be valid - health and family problems -

Is there any hope to get a new IV number without a whole new petition?.
Is there a procedure for re-newing an unused expired Immigrant Visa
(IV)?
