is there any way of knowing if your application was even accepted to be in the
lottery (as in not disqualified)? thanks!

Is there a sample form somewhere? One where you just fill in the blanks?I made my own
for DV2003, but wonder if it is ok. INS (or whichever department do it) don't give
out a proper form.

There is no form, you just follow the instructions using a blank piece of paper
as a start.

Sure they do. The instructions give an example. Just follow thta example.

Here are the instructions. The process couldn't be much simpler. There is no form

If the application is incorrect in any way my guess is that it is simply thrown away.
