I got a birth certificate from my native town municipal corporation where i was born.
But the certificate has only father name, date of birth, place and its in Hindi (
local ) language. My questions are:

1.Is it required to have Mother name along with Father name on the birth certficate.

2. Does it require any other information except my name, date of birth,place,
father(mother) name.

3. As this is not in english, I can have it translated in english and have it
notarized and attaching photocopy of original certificate should work for
485 filing.

In general, INS wants to see the father's name and the mother's name. However, I
don't think INS will count it against you if the official birth record does not
contain one parent's name. In that case, I think it's a good idea to supplement the
birth record with an affidavit in lieu of birth certificate.

If I were you, I would do what you described. I would also submit an affidavit in
lieu of birth certificate, executed by your mother, if possible. If your mother is
unable to sign such an affidavit, your father can do it, or anyone can do it who has
