AP form I-131, which version?

I am wondering which version of the AP for to use. The INS site says
12/10/91 is the only edition authorized for use. It looks like crap, a
photocopy.
I requested the form by mail and they mailed me the form from '98. There
are also some nice fillable's of this version on the web. So, now I'm
confused. Will they take the form from '98 even though their site says
differently?

Thanks

Use the one mailed. Web sites are not always updated.
This is an opinion only

I would (and have) use(d) the one on INS's website. the date stamp on the
page shows last updated in July of this year, which isn't that long ago, and
it is also the correct revision date. The one you received by mail is NOT
the correct revision date, even though they mailed it to you...and according
to the website it will not be accepted. why take that risk?
yes, it looks like crap but hey, theyre the ones that posted it that way,
and theyre the ones that have to read it!
I have a fillable version of the 1991 revision...if you want a copy, I will
be glad to email it to you. (still looks like crap, but at least you can
type your answers neatly.)

Thank you. I would be very grateful if you would email the fillable 1991
version.
