AOS in SF questions

My husband recently became a US citizen and now we're gonna file for my adjustment of
status (in San Francisco)
I have a couple of questions:
1) Does he submit his original certificate of naturalization or a copy? How do we
make a copy, if allowed? In general, do I leave with them the originals of all
documents or just copies?
2) I've decided to use my maiden name as my middle name, and my husband's last name.
In that case, will the full name show on my EAD permit and my green card, or just
First name Middle initial Last name.
3) Since I don't have an A number or a SSN, do I leave those fields blank or
write 'none'?

I-130. Unfortunately their search capability is broken so I can't find it. You just
photocopy it though.
