Form I-129 with the E supplement may be found on the USCIS website.

These will vary from state to state and city to city. Ingo had a recent
general post on this topic.

In general, if you have employees, you will need a Federal Employee
Identification Number. Every business will have to register to pay taxes in
the state in which it is located. In California the agency is the Franchise
Tax Board. Then, most cities have some kind of licensing requirement.
