I was wondering anybody from germany can answer this.
My wife was born in Barmbek, Germany
I dont know what the district or province Barmbek is in,
(City/Town, District/County/Province, Country )
(Barmbek, ..................., Germany)

This question might sound stupid but on the form,
that you fill, do you put the title too or just the information,
Is it going to look like this?

Name: _______________
Dateof birth:_________________

or only the name like

Kjhgfaesd Liuyjhgds (last name underlined)

Use the Bundesland that it is in. There may be more than one Barmbek in
Germany. If it is the one I'm thinking of, I believe it is in Hamburg.

Either way is fine, but I'd put the title of the field, too.

Well, Barmbek is a district of the city of Hamburg, so with the form
City/Bundesland/Country I think it should be Hamburg/Hamburg/Germany.
Hamburg is a city and a Bundesland at the same time.

Provided we are talking about Hamburg-Barmbek, shouldn't it perhaps be

Hamburg, Hamburg, Germany (Hamburg essentially being both a city and a
state)?

For reference, see the postal code search engine provided by the German
postal service
(http://www.deut-
schepost.de/postdirekt/produkte/plzsuche.html). If you
look for "Barmbeck", you'll get

Municipality
(Old Name) (New Name)
Barmbek-Nord Hamburg
Barmbek-Süd Hamburg

Following the logic that one is supposed to give the current name of the
place of birth (as opposed to the name when one was actually born), it
seems Hamburg would be the correct choice. (For comparison, my dad is
supposed to write "Lviv, Ukraine" instead of "Lwow, Poland", IIRC.)

Disclaimer: I am not at all familiar with local Hamburg customs. Locals
may have better insights.
