That can mean anything from severe concussion (which can be very serious) to actual brain damage. From the statement they are optimistic about his prognosis I would suggest the former.

calm down matey! thats a bit rough - I mean its bloody sad when somethig like this happens and I do love Top Gear. Hope the guy recovers and his injuries arent serious in the long run.
