Default Should I tip my dentist ?

Paid him with folding money in a white envelope but when I got home I thought maybe I should have tipped him to avoid agony on the next visit. Would not dream of doing it back home of course but what's the done thing here ?

I've lived here 22 years and have never tipped (nor would ever think of tipping) my dentist!! Heck - my dentist has gotten enough money out of me over the years - that's why she drives a BMW and wears a gold Rolex watch (and I don't think it's one of those fake ones). The only people I tip are servers in restaurants and the woman who cuts/colors my hair.

That is, pay your insurance co-pay, or your expected co-pay, but let them bill your insurance for the rest. In many places it is illegal for them to go after you for the part of the bill that your insurance will not pay. Tips are when employers do not pay enough and you are expected to make the difference up. Many dentists are self employed and those that are not, are not on minimum wage.
