At Heathrow and most other major airports, the fuel is piped directly in. There are then underground pipes that feed each stand. A vehicle is used to hook up the the underground system, and pump the fuel into the aircraft. Memory is getting a little hazy now, but I don't think a 45 minute operation to fuel a 747 would be unusual.
When I fuel aircraft using a truck, it's pumped in at about 40psi. Whether it's the same on the fuel bowsers that fuel the big commercial aircraft, I don't know.
