How about squeezing the patients into clothing a couple of sizes smaller so they can remember what it was like to fit through the door.

It only teaches them about handling the extra girth, not about handling any extra weight as the "fat suits" don't seem to weigh all that much, so it would be a "normal weight" person plus a little extra for the suit.
