How do you respond to "how are you?"

Most annoying: 1 particular branch of Wells Fargo:
No matter the time of day - they're trained to ask:
"And how has your day been for you so far?"
And then stop to actually wait for your reply before getting on with whatever you're there for. Crazymaking

Stock answer "good and you?"
Seems to me "how are you" is the American version of saying hello. They don't want any details, they are just trying to be polite. Prefer hello myself.

I generally just say 'Fine, thank you, and you?' I realised yesterday I've been here far to long, I was in Publix car park and asked some guy if I could steal his cart (not trolley, I got out of that habit a while ago!) as he was just finishing loading his car, and as he passed it to me I caught myself saying, 'Thanks, have a nice weekend'! Aargh!!

I've noticed this in many American programs on TV especially those series involving the "Real Housewives of New Jersey" or Orange County or Wasington DC or wherever.....all these glamorous "dressed to kill" women forever meeting up with each other, greeting each other with an embrace and a "How are you?" thingy with all the apparent sincerity of an amoeba........for two minutes later they are engaging in one almighty verbal bitchfest with each other.
My response to an American and his/her "How are you?" routine would be something like "Well, tell me - who is it who'd really like to know then?"

Last Tuesday 08/04/14 my partner and I went to London's Gielgud Theatre to see Angela Lansbury play the part of Madame Arcati in the latest stage production of Noel Coward's "Blithe Spirit" - at the age of almost 89 her performance was nothing less than absolutely fantastic (London is actually the city of her birth in 1925).
Sitting next to us was this young American girl of 21 currently in London auditioning for a place at the Italia Conti Stage School here.....she was very pleasant but gabbled away like a machine gun in typical American style, using many of the stereotypical American expressions - she was interesting to listen to nonetheless, but when we parted company at about 23:00 after another brief chat in the theatre foyer she said "You have a good evening now!"
Maybe she thought we were off to a nightclub.
We wished her a good night.
