If you become tax resident in Spain, yes, you have to.

Even if you pay no tax you would be advised to make a zero tax declaration so that you are registered into the tax system. Failure to do that could make it difficult to claim certain exemptions for things like CGT in the future.

You will never qualify for free prescriptions either as that's based on income which they can't know if you don't make a declaration.
Everyone should make a first declaration and where appropriate (worldwide assets in excess of €50k) a Modelo 720, worldwide assets include things like the value of private pensions.

I made a first declaration together with my wife in our first tax year after arrival and again for a couple of years after that then the local office said only to make a declaration if circumstances changed. We registered our S1’s and are in the health system. We each have mild hypertension mine diagnosed in U.K. before we arrived and my wife’s detected here after an event which demonstrated just how good the Spanish healthcare system is. Our monthly prescription for two packs of tablets costs €0.30 renewed once a year for the next 12 months.

I made two tax declarations ,then I was also told it was no longer necessary unless I came into some money ( chance would be a fine thing!)
As for charging for prescritions, given the excellent service we have received from the Spanish health system , I gladly pay the small amount they ask for prescription medecines.
