The best course of action would be to contact an agent for the insurance company of your choice and ask them what they mean by the sentence 'Eligibility: Malaysian and Permanent Resident of Malaysia'...

Your company tried to be helpful in giving a an English language monthly payment slip. But for your M-form it would have been more convenient if they had given you a Dutch version....
