Once you have a tax number you get a request to file. I've not looked into whether as in the UK there's a penalty for failing to do so as the task is not onerous.

My understanding is at least for peninsular Malaysia they're banning based on passport. i.e. If you hold a passport from a country on the verboten list then irrespective of how long has passed since...

That's rubbish, at the moment I don't think I LTSVP have a path to exchange although my wife as a returning Malaysian may have, however, I do use a bike here.

Unfortunately not, there was a foreigner ID at one time. It would certainly make my life and I suspect many others so much easier were Malaysia to issue too long term residents.

I went to the police to report an accident. Did not have passport when officer asked, it was at home, he just asked that I bring it in later when convenient. I took it back next day to show, no drama...

Last week actully. Note you have to register for a tax number first so do that if you have not done so.
I did not do online, I went in person, had to make three trips so you can save yourself one....

Depending on what you need yep a Commissioner of Oaths, cheapest at ~RM10 or so, or a notary public that will be ~RM60. I had to get some documents signed when I started to draw my pension last year...
