There's nothing to stop you booking your appointment - in fact I normally book mine before the LCA is even approved.

The consulate won't care. My first 3 positions were 12 months, 18 months, and 12 months, and they didn't even bat an eyelid.

The old visa will have both the company name and the LCA number under annotations. Thus it will NOT be valid unless you're still working at that employer and under that LCA number.

Presuming you're applying in Australia and have a standard Australian University degree then you shouldn't really need anything - or at least I've never been asked and I've never heard of anyone else...

Presuming you mean your Australian license, technically, no. Each state is different, but in general once you've been a resident for a set period of time (~30 days) you need to get a local license...
