1) how many photos do I need?

The first page of the DS 160 instructions tells you to bring a 2" x 2" photo taken within the last 6 months so I would take the same photo as you uploaded to the DS...

I did think they would write " Cancelled without prejudice " across old visa but no, there is nothing indicating it has been cancelled but as you say it does not matter anyway.

I think he means he dropped it off at a DX hub near to him and he will get an email informing him that his passport with visa inside will be available for collection from the same DX hub some days...
