Looking at this I think the fitness to fly ( which I have had done for pets in EU travel goes into clinical exam in passport - l) is probably due to the airlines requirement as it says 'no clinical...

There is no section in pet passport under clinical examination that is not under 'authorized vet' so I imagine that yes you need a certificate of fitness to fly instead -

I am not sure if the regulations coming from USA to UK are covered by the EU pet passport scheme

have a look at this site ( a pet relocation that a friend of mine used ) it may have details- also...
