The fee is fixed; any difference is based on the exchange rate the Home Office is using.

If he has remained in the UK working then that should be fine providing he has been earning above the threshold of £18,600 for the last six months. If you intend to meet the financial requirements...

Providing the salary is at least £18,600 pa and you have been employed for at least six months earning again at least £18,600 abroad then a job offer is sufficient providing it starts within three...

It's for salary threshold for Tier 2 visa holders seeking ILR. The financial requirements for family visas will be unaffected.

Read it again as some of the terminology is confusing - ILR for main applicants is already £1500 and will be going up to £1875.
