I've seen some people refer to duvet covers as just duvets. They therefore might put a duvet on a comforter or they might just use a comforter like a quilt on top of the sheets.

Comforter = nice fluffy duvet
Shams = fancy pillow cases
Quilts = padded blanket like an eiderdown
