Hi, I had to do the same thing a few years ago and I would say download it, fill it in then post it back to them. I think the NMBWA seem like to have the real thing in front of them!

Hi there, singapore airways do have movies on board but but most of the flights do not have them on demand (unless you get lucky and have a brand new plane!). Hope this helps.
