Don't know the details (I can find out) but early retiree friends of mine moved to Spain just under 2 years ago and were able to join the health system without any particular difficulty.

You can register with the Certificate of Permanent Export, I know because I've done it, coming direct from DVLA and naming you as the keeper it's actually a more authoritative document than the V5.
