As far as I know, as an immigrant you can't just change your last name like citizens here. Everything has to go through INS first.

I wrote mine using the same format as I-129F petition cover letter. Pretty much a list of the things that we include with the petition. I just like to be organized :)

Try the best you can to contact someone at the consulate. They'll tell you which tax return you'll need to present to them. This is the best way to do it. I did this myself. Keeps you in the right...
