I don't think it matters. Your A number is on your green card, so they can match it up. I think the case number was on the yellow sheet too, but I don't remember. In any case, I wouldn't worry...

It a form letter. Funny thing is, they just updated the I-751 form to include the photo requirement, but they didn't updated it to include the original green card. Maybe it's a temporary thing.

I see now that they announced the fee changed on 9/26. You have to look for it...

I called the national line, and they were no help what so ever. They haven't even received the application yet...

Your AP should have 3 pages. Actually 4 with the cover page.

1 is for the carrier - they usually don't take it because they don't know what to do with AP.
2 are for your reentry. We used it...

I wouldn't worry about this year since they are still working on petitions from last year... I stopped waiting months ago. It's too stressful to have some hope everyday when I go to the mailbox.
