This NEVER happened to me. My exam was a series of questions about drink/smoking/drug use, my and my families medical history (any history of cancer, etc) and then they took blood pressure, checked...

This may be urban legend but I was informed by a colleague that when one leaves the US permanently at (or before) the expiration of their H1-b, they are entitled to a refund of their medicare...

I think this is called 'benching' and is not allowed under the rules of the H1-B. And since you have been 'on the bench', so to speak, for nearly two years I would think it would cause a problem...

Absolutely do not do that. Working whilst on the VWP is strictly forbidden so you would not be able to start work for your employer anyway - even entering the US on the VWP with the intent to work...

I don't think there is an allocation of H1-Bs reserved for people that get their degree in the US. The quota is open for anyone with a degree from an accredited university, regardless of it's...

A B-1 maybe? Since it's not an accredited educational institution in the tradtional sense I don't think an F-1 (student visa) would fly.

The expiration date on the visa that is pasted into your passport is, in essence, the last day you can apply for entry to the USA.
How long you can stay after that is governed by the expiration date...
