Not necessarily - there may be valid reasons which ECO will accept. Even though ECO are strict they do have discretion to use compassion. A strong cover letter explaining all situation is required.

It hasn't even been an year since you last visited your wife, shouldn't be a major issue. Make sure you have call logs etc to cover the time apart.

If you are relying on your current house shared with your sister then don't complicate things by submitting docs regarding looking to buy new house. Keep it simple.

There is no specific requirement for it to be in a separate account but would be better to have a separate savings account. You can explain it in the additional information part of the application.

Yes. If its 2 bedroom and no other adult lives there that's all you will need. Perhaps add a utility bill too. However if there are other adults living there, you should get a property inspection...
