I think you need to send Form 1022 (Notification of changes in circumstance). after baby is born.

If your visa is not granted at this stage,they might add your new family member...

Every one needs his own passport for visa to be issued.Only make it clear if you have option to provide child's own passport during process of application.Otherwise they will not...
