The airlines are quite clear on what is allowed and isn't allowed; to answer both your points relating to that, you most certainly can control your environment. If you don't want to go on an airline...

Our vet actually disagrees with you - hence our taking the cats in the cabin. He strongly advised taking them in the cabin, or not taking them at all.

We are flying our cats London-Frankfurt-Los Angeles with Lufthansa on Tuesday in the cabin. (We are moving to LA, so it will be a one time deal, hopefully!)

We couldn't find a direct carrier...
