alerts:

New CIO checklist Schedule A only asks 10 past years, NOT since 18 years old.
ALSO, payment form is new one dated on 01-12, eventhough same as old one. Make sure use the new form.

Blue, have you submitted your application to CIO? It took 3-4 weeks for CIO to do quality review, if passed, the charge will be exercised, which is a good news.
