US passport and Indian Visa

My son has US passport and Indian Visa (10 years).

I am on h1.

Apart from US passport and Indian visa stamp is there any other doc required to travel to India for my son?

I have sent my old I-94 and copy of new I-94. Will it be a problem for stamping ??

For revalidation stamping to send docs to St Louis,

should i send I94 which i got when entered US

OR

I94 which came along with extension I-797 attached at the bottom.
