Don't you have their business card? Write the name of the person who was dealing with you and also mention the business name. I guess leaving MARN field empty and mentioning that you don't have it...

You need to send only form 956 in order to notify DIAC about the exemption of your agent. I don't think that you need to attest the form and yes i think it should be ok if you send it via email with...
