Thanks Rene. I did submit it with my AOS paperwork/package - hence why I had assumed that it was a standard letter format etc.

The I-129f, yes.

The lewisville address/Dallas Lockbox for courier as per http://www.uscis.gov/files/form/i-129finstr.pdf page 5.

She is currently in the US, although has a Tier 1 visa for here.
