Yeah, you'll want two copies of the birth cert (and maybe of 156, 156K, 157 and 230 if you havent already) other than that it seems everything is in order.

As far as I can tell, ESTA registration is only needed if you're travelling under the Visa Waiver Program. If you have a visa, then you don't need to ESTA.
