Put your national insurance number for tax and social security I think as I think they are linked, with the id I put passport number as far as I can recall.

IMHO go for the full assessment as they do not recognize the pre assessment, having done this myself it was relatavely easy to do (although the outcome was not good in my case and I am fighting them...
