With regard to the extension letter, I thought that this meant it applied to the card.

However, you speak to USCIS rep and then they'll tell you it from the date of the letter.

From my experience of obtaining an L1 (and this was 3 yrs ago) your biggest time lag is getting your interview date with the US embassy in London.
It normally takes 2-4 months for a regular L1, but...

I got a receipt notification, then had the biometrics done on Aug 1st. Since then I've heard nothing. The USCIS website (Vermont) doesn't show the correct processing times so I hope my 1 year...
