Air India and Saudia for me.

Saudia are the worst I have ever flown, routinely cancelled flights, hours late, bumped because some local wanted a seat.
Then to top it all off I remember reading a story that they were being charged with using donkey meat for beef inflight.

I'm surprised South Africa and Thai are there.

I flew Turkish a couple of times. Thought it was OK.

This list seems to base it's score on lifetime safety records, as well as disregarding cause and blame. And yes, curious case about SAA, their only hull loss is unresolved and steeped in conspiracies, but don't know much about their safety record elsewhere. Flew them alot in the 80's. Korean Air is generally appreciated for improving their safety record over the past decade, but why no Air Koryo?

Flown on 4 of them.

Oops, been on 1,3,6,7,8,9 in the last two years.

Does that make me lucky to be alive?

Tower Air - Paris to JFK was the worst I ever flew.
Egypt Air and Syrian Arab Airlines weren't much better

Mate, they're first class.

Try Sempati Air (Indonesian ex military) in the late 90s. I tell you the stewardess was literally holding the door shut.

Thank some omnipotent being that it was just an hours flight...

Did myself - was holding the illuminator shot on SCAT AN-24 night flight over Caspian and Caucasus.
I was happy - at least I had a seat. Due to heavy overbooking many must sit on their bags in the aisle.
This was regular passenger flight, just few years ago.

And not forgetting the Concorde, 109 dead. Plus a bunch of other serious incidents (including a hull lose with no deaths, and a death from a projector screen and turbulence incident) and hijackings over the past 30 years.
