MBT's can be good but shouldn't rely on them to improve your posture. There's a lot more to improving posture than wearing a pair of shoes. Quite a few of the chiropractors I used to work with swore by them. However if you have a back problem I wouldn't recommend wearing them unless you have advice from a professional.
As for a mattress it shouldn't be too hard or too soft, and the top layer should be enough to support you. Orthopaedic mattresses can be too firm so it's better to go for memory foam or pocket sprung. And then it's all about preference as to which you feel is more comfortable. Sleeping position is also important and a lot of people complain about their mattress when in fact it's the position they sleep in causing the problem.

I live in flippies or work shoes.
My back hurts because I'm fat, not because I'm in the wrong shoes.

There are plenty of shoes that are better for your feet - slippers, trainers etc are much better than flip flips. Even bloody crocs (I can't believe I said that).
