The main point of the medical exam is to check for various genetic illnesses such as thalassemia. If both are found to have the gene for thalassemia they are advised of the risks for any children and what they will need to do. The original idea was to discourage the couple from marrying but its had almost no success. As a result and due to extreme consanguinity a rare ressesive condition is at the highest levels any where on earth and uses about 50-75% of all blood supplies in the country.

it's how Singapore and Malaysia also quietly get around the rules - that or a quick trip to UK or Australia if you can afford it
