If you are looking to keep the cost down then maybe see if your insurer offers what ours does. We have a “Major Medical” policy when means that the first US$1,000 on any new claim is down to us.

I think that there are a variety of policies available here that can meet the requirements of MM2H. Some are the low cost, high deductible, high co-pay policies that are essentially tissue paper coverage. Then there are real policies that provide decent coverage in Malaysia. Then there are the premium policies that offer both Malaysian and International Travel Coverage. Some of these may be offered by the same companies. The more substantial policies will also ask for health history, checkup, and scale cost to age/lifestyle (smoker, weight, etc.). I think it's hard to direct people to a particular insurer or policy (except for the really cheap ones that just barely provide any coverage) unless one knows quite a bit about the customers needs and could actually look at the actuarial tables that the insurers keep locked away. Probably best to shop around, let the agent know what you want, get a quote, and the compare that with others you do the same thing with.
