Holy cow, that's a ton of traveling for a 20-day trip. If you have 5 extra days
to kill I'd recommend just staying put in one place or you're going to totally
wear yourself out. Cities relatively close to Vienna include Prague, Budapest,
and Bratislava if you're interested in adding some Eastern European flavor to
your trip though.

The Vatican is in Rome, so those can be done on the same day, freeing
some time for more destination. Tromsø, Athen and Lisbon are supposed
to be worth checking out.

I'd suggest you spend those "few days" resting for the ordeal to come!
8-) How can you expect to really see anything at all, trying to cover
that much territory in only twenty days?

You might consider Budapest. I had a great time there last year.
They are very welcoming to tourists, and there is much to see. Good
cuisine, museums, history, architecture, etc. From there, a
convenient train trip or boat ride to Vienna....

Don't listen to the others, they are just jealous. Here is my
suggested itinerary:
Day 1:Vienna/Salzburg/Innsbruck (seeing train stations only)
Day 2: Rome/Vatican/Florence/Pisa (no more than 3 mins in each)
Day 3: Nice/Switzerland (writing postcards on the train between)
Day 4: Cologne/Heidelberg/Evian (You'll need amphetamines at this
point)
Day 5: Brussels/Amsterdam/Hague (top up the amphetamines in Ams)
Day 6: Paris and London (pick up postcards to authenticate the
experience)
Day 7: Copenhagen/Oslo/Stockholm/Helsinki/St Petersburg/Moscow
Day 8: Side trip to Africa (any country, they all smell bad)
Day 9: Side trip to Asia (make certain to pick up some dysentry to
take back with you)
Day 10: Go home early. Congrats, you've done earth.

Re: advice needed
Beware, with just 20 days, you're going to spend the entire time
unpacking/checking-into-hotels/packing/checking out of
hotels/traveling to the next location. The only time you are going to
have to see anything will be on the trips between locations.
Do the math:
8 hrs = sleeping
1.5 hrs = breakfast/lunch/dinner (1/2 hr for each, if only fast food)
0.5 hr = bathing & the necessary
0.5 hr = check in /out time
4 - 6 hr = travel time minimum not necessarily realiistic between some
of those locations
2 hr = tour bus around the city (includes waiting ro tour bus to
arrive)
totals 17 hrs (more or less)
17 hrs x 18 cities = 289 hrs
289 hrs + 24 hrs (2 half days lost arriving in europe & departing from
airports, including customs) = 313 hrs
313 hrs versus 480 hrs (20 days x 24) gives 167 hrs or 9 hrs 17
minutes to soak in the cities.
9 hrs isn't much time to enjoy the places.
Don't get trapped in any lines waiting to buy tickets or maybe just
use that time to catch your breath.
The only way that this trip makes any sense is if you are on one of
those tours modeled after the movie "If it's Tuesday, this must be
Belgium". And I'd, personally, still avoid it...
If I was going to rush around and never come back to Europe, ever ever
again, I'd still want a minimum of 3 - 4 days to even do more than
just sneeze in each city and send post cards home.
This trip reminds me of the question I was actually asked "Should we
see the Grand Canyon in the morning, Disneyland for lunch & go to San
Francisco for dinner or should we do it in a different order?" If you
know the distances involved, you'd see the silliness of this question
Disneyland to SF is an 8 hour drive.
I'll admit that things are a lot closer together in Europe... but
don't you want to see Europe before your holiday snapshots are
printed?
No matter what silly advice you may get, please enjoy your trip!

Actually with 18 cities in 20 days (does that include the arrival and
departure day?), they will probably never even need a hotel...they'll
be living on the train and whatever food they can scrounge at station
stops. At least they can probably pick up some postcards and tacky
souvenirs at the stations to prove they were there.

You *are* aware that you're going to have just the evenings to see each
place, as you'll spend a good portion of each day travelling to the next
destination.

Yes, 18 locations in 20 days is
stark staring mad.
1/3 of it would be a tight schedule
