Hello everyone, I am in need of some help. I am struggling to understand how to fill it in (my Portuguese isn't that good at the best of times) Really I just need to know what goes where. If someone could please help as I'm getting stressed about it (my job starts on the 19th of August) I have tried asking the relevant department in Portugal a couple of time's and they still haven't come back to me =( I have attached the form, it's in pdf format, and a copy of some instructions i got sent at the same time.

I may be quite wrong here, but the application form seems to be intended for persons (portuguese or not) that require a criminal record from the Portuguese Judicial System, thus pertaining crimes committed (or not) in Portugal, is that what you want?

I would venture that you need a criminal record issued by the country you are coming from, not Portugal?

Thanks for your comments =) I need it as I spent a year in Portugal a couple of years ago and I'm heading back to work there. I work with children and young adult's as an outdoor instructor so I need a 5 year criminal history. I'll be checking out the group you suggested =)

hi Alex,

If you want a criminal record, and you are in Portugal, you can go to a "Loja do Cidadao", and you will get it on that moment, if you don't have any issues.
If you are in other country you have to send the form to the address that they give.

I complete the form with the information you have to provide. I don't know if help, but I hope so.
