By issuing it means who pays. I cannot understand how this situation has arisen.
UK health is based on residency. (unless you are a resident of another country in Europe)
But then only if you are already in that country.
As such any Brit becoming a resident of an EU country, after 231st Dec, will not have the benefit of an EHIC, unless individual country by country is agreed.

Who does?
It's a legally binding document, signed by both sides, for which there are serious repercussions should it be breached. It can't just be ignored or revoked.
And even if not in the WA, there were documents published on government websites guaranteeing specific provisions in permanence to people in various situations. Do you think the government can, or should, trash those promises?

Just tried to get a new UK EHIC and told I'm ineligible. So therefore no type of medical aid after 31/12/20 unless you're out of the UK on 31/12/20?

They published explicit statements guaranteeing future status on pensions and healthcare for UK citizens already resident in EU countries by the end of the transition period.
There is a considerable difference between saying "We would like to continue with x" and "As a person who fulfils the following set of conditions, you will remain eligible for life for x"

The new EHIC is for certain people who are resident in the EU (but not the UK) and the current EHIC is for those currently resident in the UK only. The current EHIC will effectively expire on 31/12/20 or when your holiday ends (if you're on holiday on 31/12/20).

The UK government expressed a desire to continue with reciprocal EHIC arrangements for all post-Brexit. That may or may not eventually be fulfilled.

My guess is it that come Easter and beyond into summer when people are booking holidays and it dawns on them that they are now not covered by an EHIC, but find out that some people are covered (who by the way pay little or no taxes in the UK), if not already resolved between UK and individual countries, the social media and the general media will have a field day. I think that social media has a bigger influence on Gov policy than they seem to think.

What Marcus Rashford was asking for was entirely in the government's hands to concede and didn't involve them breaking a previous explicit guarantee.
I'm asking you what it is you might expect the government to do under the hypothetical circumstance you outlined, as it's not clear from your previous post.

It's probable that premiums will go up substantially too, as the current level prices in the reciprocal arrangements. Some higher risk folk may have problems getting cover, as is currently the case for travel insurance to the USA.

This means after 31/12/20 your old NHS hospital is at liberty to turn away uninsured non-resident Brits. Non-resident Brits will need travel health insurance to visit the UK.
(The NHS has not typically enforced this, but that could change. If something serious happened, you've have to have proof of residency, such as NI contributions)

UK-funded healthcare using an S1 from 1 January 2021
If you’re living in Portugal before the end of 2020, your rights to access healthcare will stay the same from 1 January 2021 if you’re either:- receiving a UK State Pension
- receiving some other ‘exportable benefits’
- a frontier worker (someone who works in one state and lives in another)
- continued access to healthcare in Portugal using your UK-issued S1 form
- a UK-issued EHIC for travel
- planned treatments in other EU countries via the S2 route
- access to the NHS in England, Scotland and Wales when you’re visiting the UK
