To apply this to your question OP, you don't get full health coverage under the EHIC system but you get enough to prevent you from having to return home under most circumstances.

In other words, you get as much as you can without actually being resident.

Hope that explains sufficiently.

BTW - don't get too carried away with the idea that Portugal's bureaucracy is bad or worse than anywhere else. It's one of those recurring (but not necessarily true) themes on expat forums.

Eric you do have a habit of leading posters up blind alley ways.

I repeat it states clearly that to qualify for an EHIC you have to be ordinarly resident in the UK.

If in receipt of a state pension then the S1 rules apply.
