We have our second Astra-Zeneca jab booked for Wednesday. Even if I feel as bad as after the first one it will be worth it.

This larger gap seems to have imparted a much stronger response. So a top-up may or may not be necessary. The initial talk was that any booster would be tweaked to better fight the variants but that seems to have been dropped in favour of a general strengthening of immunity. There is also the school of thought that once vaccinated any introduction of covid to the system would in itself act as a booster. I guess time will tell.
The other recent, complementary line of thought, voiced by Dr. Anthony Fauci in the US, among others, is that those with compromised immune systems are unlikely to achieve typical protection levels from standard doses (whether one or two) of any of the mainstream vaccines currently semi-approved (FDA emergency authorization) for use in the US. A further emergency authorization is expected to permit booster shots for that immune-compromised population. (But I've had a hard time finding a definitive list of conditions ruled in or out of "immune-compromised".)

That's US-centric news, to be sure. But eventually it's likely to guide the thinking of authorities elsewhere.
