Default discharge planning

what are the services available over there in Oz for discharge planning

we refer to social services on admission so that the discharge goes off without a hitch also district nurses

what services are there for this in Oz and who pays?

Hi Asher,

We have discharge planning on our ward from the moment the patient arrives, we use a computer system called JONAH which was chucked out by the UK. Baically all allied services get together to be involved in the patients discharge if necessary via a nurse we call a clinical care coordiator (every ward here has one) she liases with everyone and faxes district nurses ect prior to discharge, social services not evident here as far as I can see, but this is a very different system to uk. Patients are not registered with doctors so can see any doctor they like, district nurses not assigned to any doctor so work from the hospital as in public district nursing or from home as in private district nursing usually in areas and not from health centres like in the UK.

Redcliffe Caboolture QLD, have discharge planners except they are called CHIP nurses and get NO2 money.
